Act as liaison; facilitate and improve communication between client and providers/care community.
Identify, coordinate, and connect clients with community resources & services. Discuss available program offerings and how to best utilize.
Coordinate medical services, attend medical consults with client, assist with complex medical decision making, arrange second opinions, secure services from other medical systems, research and explain treatment options/risks/benefits. Advocate to achieve a definitive diagnosis;
Elder-Care Guidance, Residential Care and Community Placement solutions. Present options, discuss pros/cons, affordability factors, values and priorities of individual/family.
Discharge planning; making certain supports, equipment and community services are in place.
Advocate for client regarding providers’ billing and insurance.
