Services Offered, But Not Limited To

  • Assessment   of needs and strengths of you, your loved one and your family.
  • Achieve  a definitive diagnosis; coordinate medical services, attend medical  consults with client, assist with complex medical decision making,  arrange second opinions, secure services from other medical systems,  research and explain treatment options/risks/benefits. 
  • Elder-Care Guidance, Senior Residential Care and Community Placement. Present options,  discuss pros/cons, affordability factors, values and priorities of  individual/family.
  • Offer  guidance, support and education to clients, their families, and caregiver. Assist in creating a care plan and management.  Support clarification of personal/cultural values as it relates to  their care-plan. 
  • *Evaluate and develop effective plan and solutions for client’s care while  encouraging client involvement.
  • Act as liaison; facilitate and improve communication between client and  providers/care community.
  • Identify,  coordinate, and connect clients with community resources. Discuss  available program offerings and how to best utilize.
  • Discharge  planning; making certain supports, equipment and community services are  in place.
  •  Advocate for client regarding providers’ billing and insurance.

Over the phone or in person. 

To determine your main concerns and make an initial plan.
